What is implant-grade titanium?
Implant-grade titanium means titanium certified to a medical implant specification — most commonly ASTM F-136 — verified nickel-safe, lightweight and suitable for healing piercings. 'Titanium' alone guarantees nothing: only a stated, certified grade does. It is the default material professional piercers recommend for fresh piercings.
A specification, not a metal
“Titanium” describes an element. “Implant-grade titanium” describes a contract: the metal has been made, tested and certified to a standard written for material that lives inside the human body. The specification you will see most often on good piercing jewellery is ASTM F-136 — a titanium alloy standard developed for surgical implants such as bone plates and joint components. Commercially pure titanium certified to ASTM F-67 also qualifies, and equivalent ISO implant standards exist internationally.
For a piercing, the practical meaning is simple. A fresh piercing is a healing wound with jewellery sitting inside it around the clock, for months. Implant certification is the assurance that the metal in that wound is what it claims to be: consistent in composition, free of meaningful nickel content, and manufactured to a verifiable paper trail. This is why the Association of Professional Piercers (safepiercing.org) lists implant-certified titanium among its accepted materials for initial piercings.
Titanium has two further advantages that matter specifically in ears. It is roughly half the weight of steel or gold, which healing cartilage in particular appreciates — less drag on the piercing channel means less irritation. And it can be anodised into a range of colours without any coating or plating, so the surface against your skin is still bare titanium.
Why “titanium” on its own means nothing
The word titanium, unqualified, is doing no work at all. Fashion jewellery is routinely sold as “titanium” when it is titanium-plated, titanium-coloured, or an uncertified alloy of unknown composition. None of that is fraud in most cases — it is simply that nobody checked, because nothing requires anyone to.
The grade is the entire claim. A studio or retailer who says “it’s titanium” has told you nothing; one who says “it’s ASTM F-136, and we can show the mill certificate” has told you everything you need. If the person selling the jewellery cannot name a specification, the safest assumption is that there isn’t one.
The “surgical steel” problem
Surgical steel deserves a specific warning, because it sounds like an equivalent and is sold as one. It is not, for two reasons.
First, steel contains nickel. Even the genuine implant-grade steels used in medicine are nickel alloys; they are engineered to release very little of it, but the nickel is there, and for the many people with nickel sensitivity that is a meaningful difference from a nickel-safe titanium alloy.
Second — and more importantly — “surgical steel” is not a specification at all. It is marketing language with no regulated definition, applied to everything from genuine implant-grade material to cheap alloys that would fail any implant test. A £3 stud and a certified medical component can both legally wear the same label. Implant-grade titanium with a stated ASTM grade closes both loopholes at once: no nickel to speak of, and a standard you can actually verify.
What to ask a studio
You do not need to become a metallurgist. Three questions cover it:
- “What grade is your titanium?” The answer should be a specification — ASTM F-136 is the usual one — not a reassurance.
- “Where does your jewellery come from?” Studios buying from reputable piercing jewellery manufacturers can answer immediately; vagueness is informative.

A professional studio will treat these as normal questions, because they are. The studios to be wary of are the ones that treat them as an affront.
See a professional if: a piercing is reacting badly to jewellery you were told was titanium — a reputable piercer can identify the piece, confirm or rule out its grade, and swap it for verified implant-grade jewellery; and a GP or dermatologist can investigate suspected metal allergy properly through patch testing.
